      All relevant original documentation must to be brought to the ZAP Character Assessment and shown to the officiating ZAP Character Assessment Assessor. I agree to abide by the rules and regulations pertaining to the GSDCA ZAP Character Assessment and consent to the results of this ZAP Character Assessment being published in the annual GSDCA ZAP Character Assessment report and on the GSDCA Website. I understand that a false declaration will result in the ZAP Character Assessment result being made null and void and any award that has been achieved as a result of this false declaration will be revoked.
