BS1 form Applications for Breed Survey are due not less than 7 days prior to your nominated Breed Survey date. Submitting later than that may require that you present your dog to a later Breed Survey date.

    - Copies of Titles and Character Assessment Certificate (if applicable)
    - Title Verification and Breed Survey translation (for imported dogs)
    - For a represent after failing survey – attach a copy of the GSDCA Form BS 2 - Notification of Failure
    Failure to provide all of the above documentation with this application will result in the animal being unable to participate in this particular survey. All relevant original documentation must to be brought to the breed survey and shown to the officiating Breed Surveyor.

      Upload a photo image of your animal, ideally less than 1MB. If you dont have a current photo, this can be suppiied within 7 days of the survey to the NBC Registrar (regnbc@gsdcouncilaustralia.org)
      I/we submit the following animal to the Breed Survey being held on the above (selected) date with the relevant fee(1) paid and declare any prior presentation that this animal has to the GSDCA National Breed Survey Scheme. (If in doubt it is the responsibility of the owner to check with the National Breed Commission Chair). If the animal has been presented to Breed Survey please supply details of such presentation.

      I/we have enclosed copies of the following documentation, ANKC Certificate of Registration, a Five (5) Generation Pedigree, and the Certificates/Result Sheets for HD pass, Elbows pass, as applicable. In the case of an animal being represented after failing survey a copy of the GSDCA Form BS 2 - Notification of Failure must be attached. Failure to provide all of the above documentation with this application will result in the animal being unable to participate in this particular survey. All relevant original documentation must to be brought to the breed survey and shown to the officiating Breed Surveyor.

      I agree to abide by the rules and regulations pertaining to the GSDCA National Breed Survey Scheme and consent to the results of this breed survey being published. I understand that a false declaration will result in the breed survey result being made null and void and any award that has been achieved as a result of this false declaration will be revoked.

      I agree to submit a coloured photograph on the day of the breed survey, or post/e-mail a photo to the NBC Chairperson within seven (7) days of the Breed Survey.

      This application must reach the Club Survey Registrar no later than seven (7) days prior to the date of the Breed Survey. Only entries on this form will be accepted. The application form is to be retained by the Club Survey Registrar and a copy forwarded to the NBC Chairman with the GSDCA Breed Survey Worksheet
